New patients require a referral from a healthcare provider or hospital. New HIV or PrEP patients may contact our clinic directly to schedule an appointment.

To request a referral, please ask the treating provider to send your relevant medical records, including laboratory results, cultures, and imaging reports.
Once received, our clinical team will review the information and determine the appropriate appointment priority.
You may contact our office to check the status of your referral; however, please allow time for the review process and avoid multiple calls in the same day.
Referring providers may contact our clinic or an IDRG provider to coordinate a referral and discuss the patient's infectious disease care needs.
Please have your clinic fax all medical records relevant to the referral, including laboratory results, cultures, and imaging reports.
